Why is my diagnosis grayed out when I attempt to edit it?

Why we aren't able to access the diagnostic criteria in the diagnosis tab when trying to edit a diagnosis. We can click on diagnosis and hit "edit," but what comes up is "grayed out" and can't be clicked to add or remove symptoms. Often a client's diagnosis is one thing at evaluation, but at the time of treatment planning, we need to update it.

Explanation Why Grayed Out:

 The reason you are not able to modify grayed out diagnosis is that it has been used in another module. For example Admission and Discharge or Progress Notes. It now a part of other documents becoming part of the historical documentation

  

The [Delete] button

The [Delete] button will only delete a diagnosis if it hasn’t been used as part of Admission and Discharge or Progress Note. This helps if the user has just created the wrong diagnosis

 

The [Edit] button

The [Edit] works differently if the diagnosis has or has not been used in other modules (mentioned above). If it has not been used in other modules, the user will be able to change the section called DSM 5 Substance Use Criteria and Details. If it has been used in other modules, the user will only be able to change the Detail section. The DSM5 Substance Use Criteria will be grayed out.  

 

Solution – Change Status of Diagnosis

The solution is to use the [Update Status] button to change the status of the selected diagnosis from Active to Inactive. Also, you would need to remove the green checkbox marked either Primary or Admitting using the Edit button. Once this has been done create a new diagnosis.
